Once you purchase your ticket, you will receive a confirmation email with a link to sign up for your time slot.
Saturday 10 am -4:20 pm & Sunday Noon-4:20 pm.
New patient evaluations are ONLY $75 and renewals $50.
* We hope to avoid long lines and technical problems from previous years.
All potential patients will need to fill out a patient application form, bring a valid Arkansas ID (must be Arkansas) and to really speed up the process, please bring documentation of your qualifying condition.
